Advance directives feel heavy, but the structure is simple. Here is how to fill yours out calmly and completely.

An advance directive is not as heavy as it feels. Once you understand the structure — your wishes, your agent, your signature — the document is quick to complete and even quicker to update.

Decide your wishes first

The living will asks for your preferences on treatment. You do not need a medical degree — answer in plain terms what you would and would not want, and update it as life changes.

Choose your agent

Choose someone who knows you and will act on your wishes. Talk to them first. Name an alternative in case the first person is unavailable.

Sign it properly

The signature and the witness or notary block are what make it valid in your state. Follow the instructions, make copies, and give one to your agent, your family and your doctor.
